What you fill in
The form has 7 fields: fullmaktsgivare (du), personnummer, fullmaktstagare (ombud), ombudets personnummer, vad fullmakten avser (ärende/befogenhet), giltighetstid (t.ex. tills vidare eller datum) and ort. Fields marked with an asterisk are required for the document to be created.
