What you fill in
The form has 8 fields: avsändare (borgenär), mottagare (gäldenär), fakturanummer, kapitalbelopp (kr), inkassoavgift/ränta (kr), senaste betaldag, datum and ort. Fields marked with an asterisk are required for the document to be created.
